This is an affiliate application form. By default, only name and email are asked of potential affiliates. New fields can be added as custom form fields. Examples of these are to the right and include fields that map to affiliate details (PayPal Email), a populated dropdown (Country), a text box, an auto-coupon code field, and a checkbox.
These are terms and conditions. You must set the rules and restrictions on affiliate behavior, unless you have an open program.
